About NORTH POINT HEALTH & WELLNESS CENTER INC

NorthPoint Health & Wellness Center is a Federally Qualified Health Center and nonprofit social/human services agency that has served North Minneapolis since 1968. It provides integrated primary care, dental, behavioral health, and human services with a focus on culturally responsive, trauma-informed care. In recent years NorthPoint completed a historic campus expansion (completed with a ribbon-cutting on May 18, 2024) that doubled campus space and increased capacity for integrated services, food distribution, a teaching kitchen, and community programming. In 2023 NorthPoint reported serving more than 22,000 individuals in its clinic and more than 23,000 participants in human services programs; the organization also completed a leadership transition in June 2024.

Notable achievements

Historic campus expansion

Completed a community-led campus expansion (surpassing $90 million, including major Hennepin County investment) and opened a 135,000-square-foot integrated health and human services campus with expanded medical, dental, behavioral health, food shelf, child wellness and workforce-related amenities (ribbon cutting May 18, 2024).

First FQHC Health Care Home in Minnesota

NorthPoint was the first federally qualified health center in Minnesota to be certified as a Health Care Home, demonstrating a long-standing commitment to coordinated, patient-centered primary care.

Large-scale community impact and food access

Operates a Mobile Food Shelf and expanded food distribution operations that have historically reached large numbers of households (the Mobile Food Shelf has served approximately 1,000 households per month) while delivering tens of thousands of clinical and social service encounters annually.
