About Southside Community Health Services Inc

Southside Community Health Services (Southside) is a Federally Qualified Health Center (FQHC) and 501(c)(3) community health center founded in 1971 to provide primary care and integrated services to South Minneapolis. Southside offers a full spectrum of care including medical, dental, vision, and behavioral health in a culturally responsive, trauma-informed setting and accepts patients regardless of ability to pay. In recent years Southside has expanded capacity and clinical quality: it has been recognized as a national quality leader for clinical outcomes and completed a major capital project to consolidate and expand services into a new state-of-the-art East Lake Street clinic. The organization functions as an essential safety-net provider focused on improving access and health outcomes for low-income and underserved residents of South Minneapolis.

Notable achievements

Opened One Southside — East Lake Street Clinic

On May 4, 2026 Southside opened a new ~30,000 sq. ft., multi‑service East Lake Street clinic that consolidated primary care, dental, vision, behavioral health and an on-site pharmacy to expand access for thousands of patients in South Minneapolis.

National Quality Leader awards

Earned national quality recognition in recent years for clinical performance (including Heart Health and Diabetes outcomes) and ranked among top-performing community health centers for preventive care and high-value care.

Expanded survivor-centered services (SAFE Room)

Implemented an out-of-hospital Sexual Assault Forensic Exam (SAFE) Room and survivor care services in 2026 to provide trauma-informed, non-ER forensic and follow-up care for survivors.
