About HEALTHCONNECT TEXAS

Healthconnect Texas is a Houston-based 501(c)(3) nonprofit that operates a statewide health information exchange (HIE) and community information exchange (CIE). The organization connects hospitals, clinics, public health entities, and community service providers so authorized partners can securely share patient and community data to support whole-person care. In 2025–2026 Healthconnect Texas completed a strategic expansion that included a statewide rebrand (from Greater Houston Healthconnect), a unification with the Patient Care Intervention Center (PCIC) to merge clinical and community data capabilities, and the opening of new headquarters in Houston. The organization now reports statewide coverage serving millions of Texans and focuses on projects that improve access to care, support navigation for uninsured and low-income patients, and strengthen connections to housing and social services.

Notable achievements

Statewide rebrand and scale-up

Rebranded from Greater Houston Healthconnect to Healthconnect Texas (announced February 17, 2025) to reflect expanded statewide reach; the organization now reports coverage across hundreds of Texas counties and connects clinical data for millions of residents.

Strategic unification with PCIC

Unified with the Patient Care Intervention Center in January 2026 to combine Healthconnect Texas’ statewide clinical connectivity with PCIC’s community-based data and care-coordination capabilities, expanding the organization’s ability to address non-medical drivers of health.

Blue Impact grant to strengthen housing access

Awarded a Blue Impact℠ Grant (announced June 30, 2026) to fund a multi-year initiative linking the HIE/CIE network with local homeless-response systems to streamline referrals and access to housing and stabilization services in Greater Houston.
