About PACE PARTNERS OF NORTHEAST FLORIDA INC
PACE Partners of Northeast Florida, Inc. is a 501(c)(3) nonprofit that operates a Program of All‑Inclusive Care for the Elderly (PACE) in the Jacksonville area, delivering integrated clinical, rehabilitative, social and support services through its PACE Place adult day health center. The organization was approved by Medicare, Medicaid and the Florida Department of Elder Affairs to enroll participants in 2020 after initial state approval in 2018, and was developed as a joint venture originating from Community Hospice & Palliative Care and senior-services partners.
PACE Partners serves older adults (age 55+) who meet nursing‑facility level of care but prefer to remain at home; its interdisciplinary model includes primary medical care, nursing, therapies, adult day services, home care, transportation, prescription coverage and social supports. In the past 2–3 years the program has operated under AHCA licensure and has managed rolling enrollment (typically around 100+ participants at points in 2024–2026) while completing corrective actions required by federal regulators following a 2024 enrollment suspension that was released in August 2025.
Notable achievements
Medicare/Medicaid PACE approval & opening
Approved by Medicare, Medicaid and the Florida Department of Elder Affairs and began enrolling participants in March 2020 as the region’s PACE provider.
State authorization for 250 slots
Secured AHCA authorization and funded capacity for up to 250 PACE slots for its Duval and Clay County service area.
Operational adult day health center
Established and operates The PACE Place adult day health center to provide on‑site clinical, therapy and social services integral to the PACE care model.
