About COMMUNITY REBUILDERS

Community Rebuilders is a Grand Rapids–based, mission-driven housing organization founded in 1993 that works to end homelessness and expand stable housing options for veterans, families, and vulnerable adults. The organization combines direct housing programs (rapid re-housing, permanent supportive housing, veteran-specific programs) with system-level work to improve coordinated entry and data-driven decision making.

Community Rebuilders operates both direct-service programs and technology/system innovations (Community Housing Connect and the G.R.A.C.E. Network) to streamline access and referrals across Kent County. In recent years the organization has documented strong outcomes—high retention rates for people they house—and has published quarterly stakeholder reports for targeted outreach efforts that moved dozens of people from unsheltered living into stable housing during 2024–2025.

Geographically Targeted Housing Outreach (GTHO) outcomes

Quarterly GTHO stakeholder reports (2024–2025) document sustained street outreach and transitions from unsheltered settings into housing, reporting dozens of people moved into bridge or permanent housing across successive quarters and a cumulative program impact reported as 95 lives housed through July 2025.

Community Housing Connect development and beta

Led development and community-led beta testing of Community Housing Connect (CHC) 2.0 (beta testing in 2024) — a coordinated-entry digital platform built and managed by Community Rebuilders to improve referrals, assessments, and placements across the county; the platform was staged for community rollout in late 2025 and is being used to improve system-level outcomes into 2026.

Major philanthropic investment enabling system innovation

Secured a major Day 1 Families Fund (Bezos Day One) grant to scale technology and coordinated-care work, enabling investments in the GRACE Network and platform infrastructure that supported outreach, data-sharing, and coordinated entry capabilities.
