About Second Chance for Life Foundation

Second Chance for Life Foundation is an all-volunteer, patient-led 501(c)(3) organization based in the Twin Cities that provides peer-to-peer support, education, temporary housing assistance, small emergency grants and celebration/social programs for heart transplant and LVAD patients and their families affiliated with the University of Minnesota medical centers. The organization was conceived in 1986 and remains focused on “patients helping patients,” operating support groups, visitor programs accredited through Mended Hearts/M Health Fairview volunteer channels, annual educational seminars (in-person and online), and fundraising events that sustain modest financial assistance and housing support for patients during transplant or recovery. Recent activity includes ongoing educational events and support groups, board updates and volunteer leadership additions in 2023–2025, and continued fundraising (annual golf tournament and holiday gatherings) to support patient grants and services; public nonprofit filings and directory listings confirm ongoing operations and financial activity through the 2023–2025 filing years.
