OAK MEADOWS

Oak Meadows is a nonprofit senior living community in Oakdale, MN offering Independent Living, Assisted Living, and a secured Memory Care neighborhood. The organization operates as a 501(c)(3) provider of long-term care services focused on dignity, person-centered care, and activities that promote wellbeing. Oak Meadows has operated as a not-for-profit senior community since the late 1990s and is licensed in Minnesota as an assisted living facility with specialized dementia care. The community participates in state waiver programs and provides a full range of personal care and memory-support services for older adults. In recent years Oak Meadows transitioned operationally to Ebenezer (Ebenezer managed operations beginning in 2022 and formally acquired Oak Meadows effective January 1, 2025), a larger Minnesota senior-living organization; the transition was executed to preserve continuity of care, strengthen clinical and program resources, and maintain on-site services for residents.
