About LONGFELLOW-SEWARD HEALTHY SENIORS PROGRAM
Longfellow/Seward Healthy Seniors is a 501(c)(3) community-based program serving older adults in the Longfellow, Seward, Cooper, Howe and Hiawatha neighborhoods of south Minneapolis (with limited outreach in Cedar-Riverside, Ventura Village, and Phillips West). Founded in 1994, the program provides health promotion, individual support services, transportation/ride programs, volunteer coordination, and culturally specific outreach (including services for Somali elders).
The organization serves roughly 500 seniors annually and operates a mix of programming that includes Nurse Is In clinics, monthly senior social & health talks, exercise and arts classes, diabetes support groups, low-vision consultations, foot care clinics, and targeted outreach to immigrant and refugee elders. Leadership and program staff listed on the organization website include an executive director, program manager, community nurse, and a bilingual elders coordinator who support day-to-day services and volunteer programs.
Longfellow/Seward Healthy Seniors is a member program of the Living at Home Network and is funded through a combination of public and private grants, foundations, donations and local government support, which in recent years has included Minnesota Department of Human Services Live Well at Home funding and City of Minneapolis grants to sustain and expand services.
