About GRACELIGHT COMMUNITY HEALTH FOUNDATION

Gracelight Community Health Foundation is the nonprofit public benefit corporation affiliated with Gracelight Community Health (formerly QueensCare). The Foundation serves as the property-ownership and resource-support arm of the health center system that provides primary care, dental, behavioral health, and enabling services to underserved communities across Los Angeles. The Foundation supports a federally qualified health center network that operates multiple clinic sites in Los Angeles (including Hollywood, Echo Park, East 3rd Street/East Los Angeles, Eagle Rock, and Westlake North) and works to ensure care regardless of a patient’s ability to pay. In recent years the organization completed a system-wide rebrand (transition to the Gracelight name in early 2024), continued to expand partnerships and funding relationships, and participated in financing to support facility and capital needs.

Notable achievements

Successful Rebrand and Community Launch (2024)

Completed organizational rebranding from prior name to Gracelight Community Health with a community health fair and ribbon-cutting at the East 3rd Street health center, reinforcing continuity of care while modernizing the organization’s identity and outreach.

Maintained Federally Qualified Health Center Status

Continues to operate as an FQHC with Federal Public Health Service (FTCA) deemed status and receives federal and county grant funding to provide care for underserved and uninsured patients across multiple clinic sites.

Capital financing to support facilities (2025)

Advanced a multi-million dollar financing/refinancing plan to support health center facilities and operations, enabling continued investment in clinic infrastructure and long-term sustainability.
