About NORTH TEXAS AREA COMMUNITY HEALTH CENTERS INC

North Texas Area Community Health Centers, Inc. (NTACHC) is a 501(c)(3) federally qualified community health center headquartered in Fort Worth, Texas. NTACHC operates neighborhood medical homes in Tarrant County that provide primary and preventive care, pediatric and women’s health, behavioral health, pharmacy services and chronic disease management to underserved and low-income residents.

NTACHC focuses on integrated, team-based care and community outreach to reduce health disparities across its service area. In recent years the organization has emphasized chronic disease self-management (notably diabetes), expanded behavioral-health integration, and maintained public-health vaccination and prevention efforts for its neighborhoods.

Notable achievements

ADCES Accreditation (2023)

Received accreditation from the Association of Diabetes Care & Education Specialists to provide Diabetes Self-Management Education and Support, strengthening the organization’s Living Your Best Life diabetes program and improving access for Medicare beneficiaries and others with diabetes.

Secured State Grant Funding

Awarded multiple Texas Health and Human Services grants in recent grant cycles to support family-planning, preventive care and community programs, providing material funding to sustain and expand clinical services in Tarrant County.

Published 2022–2023 Status Report & service integration

Released a 2022–2023 status report documenting expansion of integrated care models across NTACHC sites, highlighting outcomes from coordinated primary care, behavioral health integration and patient education initiatives.
