About Health Planning Council of Northeast Florida, Inc.

The Health Planning Council of Northeast Florida (HPCNEF) is a regional nonprofit health council that serves Baker, Clay, Duval, Flagler, Nassau, St. Johns, and Volusia counties. It conducts community health planning, data analysis, community health needs assessments (CHNAs), and public health research to inform local health systems, government agencies, and community partners.

HPCNEF operates programs that directly support access to care (including a federally supported Marketplace Navigator program), and it produces county- and hospital-level reports, health profiles, and planning tools used by hospitals, health departments, and community organizations. In recent years the organization has led multi-hospital CHNAs, published county cancer health profiles (July 2026), and produced the 2024 Flagler & Volusia CHNA that engaged over 1,400 community members.

HPCNEF positions itself as a neutral convener and technical partner, working with major health systems, local health departments, and funders to translate data into community health improvement recommendations and implementation plans.

Notable achievements

2022 Jacksonville Hospital Partnership CHNA

Led the Jacksonville Nonprofit Hospital Partnership CHNA (published 2022), producing regional data and 12 hospital-specific reports used by five health systems and 13 hospital campuses to guide three-year implementation strategies.

2024 Flagler & Volusia CHNA

Subcontracted to guide the 2024 Flagler & Volusia Collaborative CHNA; the process included 22 stakeholder interviews and participation by 1,462 residents through focus groups and surveys to identify priority health issues and recommended actions.

CMS-supported Marketplace Navigator program

Operates a free Marketplace Navigator program (branded 1-2-3 Covered) that provides one-on-one certified navigator assistance for ACA enrollment as part of a multi-partner CMS-funded initiative to expand insurance access.
