About NORTHEAST FLORIDA HEALTHY START COALITION INC

Northeast Florida Healthy Start Coalition (NEFHSC) is a Jacksonville-based nonprofit that leads regional efforts to reduce fetal and infant mortality and improve maternal and child health. The coalition operates a coordinated intake and referral system (CONNECT), community outreach through community health workers and family partners, and the region’s Fetal & Infant Mortality Review (FIMR) process. NEFHSC works through partnerships with hospitals, public health agencies, home visiting programs and community organizations across Northeast Florida to connect pregnant people and families to prenatal care, home visiting, behavioral health, nutrition and other supports. The organization publishes annual reports and multi-year Project Impact FIMR reports and serves as a state-funded Healthy Start coalition and community convener.

Notable achievements

Expanded CONNECT intake and referrals (2023–2024)

Implemented and expanded the CONNECT coordinated intake and referral system as a single point of entry for home visiting and prenatal supports; in the 2024 annual reporting period CONNECT conducted more than 13,500 prenatal/infant screens/referrals and referred over 7,000 women to home visiting programs.

Regional FIMR leadership and Project Impact reports

Led the region’s Fetal & Infant Mortality Review process and released multi-year Project Impact community reports (including the 2024 five-year review covering 2019–2023), using case review findings to drive community action and policy recommendations.

Community outreach and partnerships

Strengthened hospital and community partnerships and grew on-the-ground outreach: community health workers and family partners expanded outreach in high-risk zip codes, conducting hundreds of intakes, educational sessions and events at clinics, apartment complexes and recovery centers to connect families to services.
