About ST. JOHNS RIVER RURAL HEALTH NETWORK, INC.

St. Johns River Rural Health Network, Inc. (SJRRHN) is a community-based 501(c)(3) nonprofit serving rural and semi-rural communities in northeast Florida. Founded in the mid-1990s under Florida statute establishing rural health networks, it supports health equity across Baker, Clay, Flagler, Nassau, Putnam, St. Johns, and Volusia counties through partnerships, planning, technical assistance and data-driven projects.

The organization functions as a convener and capacity-builder for local providers and community partners — offering program development, grant support, workforce training, outreach, and needs assessments. Over the last 2–3 years SJRRHN has led HRSA-funded work and published county-level behavioral health and disparity analyses to guide local service planning and to reduce gaps in rural behavioral health and maternal/newborn outcomes.

Notable achievements

HRSA RCORP‑NAS Grant Award (2023)

SJRRHN and partners received a Health Resources & Services Administration Rural Communities Opioid Response Program—Neonatal Abstinence Syndrome (RCORP‑NAS) award to reduce NAS incidence and strengthen systems of care, family supports, and social determinants of health in Nassau County.

30+ Years of Regional Rural Health Leadership

Established in the mid‑1990s as one of Florida’s statutory rural health networks, SJRRHN has operated for more than three decades convening local health partners and supporting county-level planning and service delivery across seven counties.

County Behavioral Health & Disparities Data Work

Produced recent gap analyses and annual behavioral health/data reports (including Putnam County and a Nassau RCORP‑NAS disparities impact statement) to inform local planning, identify service shortages, and prioritize interventions for rural populations.
