About COMMUNITY REACH CENTER, INC.
Community Reach Center is a Colorado-based, tax-exempt 501(c)(3) nonprofit behavioral health provider serving Adams and Broomfield counties since 1957. The organization operates multiple outpatient sites, school-based programs, residential services, and a 24/7 Walk-In Crisis & Resource Center to deliver integrated behavioral and medical care.
In fiscal year 2024 CRC reported serving 10,976 unduplicated consumers and has focused recent work on implementing a person-centered engagement model and aligning services with the Certified Community Behavioral Health Clinic (CCBHC) approach to improve access and outcomes.
Recent program developments (2023–2024) include opening the Empowerment Center for Adolescents (a youth emergency placement shelter), expanding crisis navigation services, and upgrading clinical and administrative systems to support integrated care across community and school settings.
Notable achievements
Empowerment Center Opening
Launched the Empowerment Center for Adolescents in January 2024 to provide emergency placement, behavioral health supports, and pathways to stable housing or family reunification for youth in crisis.
CCBHC Alignment & New Engagement Model
Implemented a person-centered engagement model and adapted services to align with the Certified Community Behavioral Health Clinic (CCBHC) model to expand evidence-based, coordinated care during 2023–2024.
Expanded Reach & Impact (FY2024)
Reported 10,976 unduplicated consumers served in fiscal year 2024 and increased community outreach via resource fairs, school partnerships, and expanded crisis navigation capacity.
