About TARRANT COUNTY HOMELESS COALITION

Tarrant County Homeless Coalition (doing business as Partnership Home) is the regional Continuum of Care lead and planning body serving Tarrant and Parker counties. The organization coordinates the local HUD Continuum of Care process, administers the Homeless Management Information System (HMIS) for the region, and provides training, technical assistance and system-level strategies to prevent and end homelessness.

In September 2024 the organization announced a formal rebrand as Partnership Home to reflect a renewed focus on transformative change through thought leadership, strong partnerships and innovative housing solutions. In the last 2–3 years the coalition has led the local Point-in-Time count and State of Homelessness convenings, managed the CoC consolidated application process (FY2024–FY2026 cycles) and partnered with local stakeholders on housing-development initiatives such as the HOME FTW working group.

Partnership Home operates as a collaborative backbone for 40+ local partners and system stakeholders, serving as the designated Collaborative Applicant, Lead Agency and HMIS Administrator for the TX-601 Continuum of Care and coordinating data, funding competitions, and community planning to expand prevention, rapid rehousing and supportive housing capacity across the region.

Rebrand to Partnership Home

In September 2024 the organization rebranded from Tarrant County Homeless Coalition to Partnership Home to emphasize a renewed mission focused on partnerships, systems change, and housing development.

Designated HUD CoC Lead & HMIS Administrator

Serves as the Collaborative Applicant, Lead Agency and HMIS Administrator for the Fort Worth/Arlington/Tarrant County Continuum of Care (TX-601), coordinating local HUD CoC competitions and regional homeless data systems.

Led regional Point‑in‑Time and State of Homelessness efforts

Led annual Point‑in‑Time counting operations (mobilizing hundreds of volunteers) and delivered public State of Homelessness addresses (including 2025 presentations) to share data, trends and system priorities with local governments and community partners.
