About HEALTHCARE FOR THE HOMELESS-HOUSTON
Healthcare for the Homeless - Houston (HHH) is a Houston-based nonprofit and Federally Qualified Health Center that provides integrated primary care, behavioral health, addiction recovery support, dental, vision, and wraparound social services exclusively for people experiencing homelessness and housing instability. HHH operates multiple clinic sites, mobile and street-medicine outreach teams, and on-site services at supportive housing locations to meet patients where they are and coordinate housing and social supports alongside medical care. In recent years HHH has expanded outreach services (including a city-first Street Medicine program launched in 2024), partnered on targeted screening and prevention projects, and maintained federally recognized health center status to increase access for uninsured and vulnerably housed patients.
Notable achievements
Federally Qualified Health Center (FQHC) status
Maintains Health Resources & Services Administration (HRSA) Health Center Program recognition and operates as the community’s only FQHC dedicated exclusively to serving people affected by homelessness, enabling comprehensive primary-care and wraparound services.
Launched Street Medicine Outreach (2024)
Established a multidisciplinary Street Medicine team to deliver primary care, behavioral health, testing, and care coordination directly to people living unsheltered across Houston, strengthening linkage to clinics and housing services.
Sustained multi-site clinical network
Operates multiple fixed clinics, mobile units, and on-site services at supportive housing locations to deliver whole-person care (medical, dental, behavioral health and case management) for thousands of unhoused and vulnerably housed patients each year.
