About HAWAII HOMELESS HEALTHCARE HUI
The Hawaii Homeless Healthcare Hui (H4) is a Honolulu-based 501(c)(3) nonprofit formed as a public/private partnership to deliver clinical care, medical respite, hygiene services, case management, and housing navigation for people experiencing chronic homelessness. The organization’s EIN (82-3190718) matches its federal tax-exempt filings and organizational profiles and the group has been tax-exempt since April 2018. H4 operates the Punawai Medical Respite and associated clinic services (Iwilei/Punawai Building) and maintains additional outreach clinic presence in Windward Oahu. H4’s model coordinates hospital discharge referrals, on-site medical oversight, and social-service case management to reduce unnecessary emergency department use and to move patients toward stable housing. In January 2023 H4 transitioned program operations into an ongoing partnership with Kalihi-Palama Health Center to strengthen clinical and operational sustainability while continuing H4’s mission and service footprint in Honolulu and community partner networks across Oahu.
