About LEGACY COMMUNITY PHARMACY SERVICES

Legacy Community Pharmacy Services is the pharmacy subsidiary of Legacy Community Health, a large Federally Qualified Health Center system in Houston, TX. It operates community and specialty pharmacy services embedded at Legacy clinic locations to improve medication access, adherence, and clinical outcomes for patients across the Greater Houston area. The pharmacy network provides fill/refill/transfer services, prescription delivery and curbside pickup, immunizations, medication therapy management and clinical pharmacy specialist support for chronic conditions including HIV, diabetes, hypertension, and asthma. As part of Legacy’s integrated care model, the pharmacy participates in discounted medication programs (including 340B-related operations and Legacy’s My Legacy RX discount offerings) to lower out-of-pocket costs for patients and to support Legacy’s clinical programs.
