About Pueblo Community Health Center, Inc.
Pueblo Community Health Center (PCHC) is a nonprofit, federally qualified health center (FQHC) and 501(c)(3) health care provider serving Pueblo and Huerfano counties since 1983. PCHC operates multiple clinic sites and school-based wellness centers to deliver integrated primary care, dental, behavioral health, perinatal services and supportive programs regardless of a patient’s ability to pay.
In recent years PCHC has expanded facilities and community programs, reported multi‑year quality recognitions, and continued high service volumes (tens of thousands of patients and visits annually), while maintaining its community-board governance and safety-net mission.

2024 service volume
In 2024 PCHC reported serving over 25,000 patients with tens of thousands of medical, dental and behavioral health visits and delivered several hundred babies, reflecting its role as a primary safety-net provider in the county.
Charles & Helen Solano Wellness Center opening
PCHC opened a new wellness center (late 2024) offering integrated services and expanded supports—including recovery resources and social services—for people experiencing homelessness and other vulnerable residents.
Multi‑year quality recognition
PCHC has received repeated quality awards in recent years, demonstrating sustained performance on clinical quality measures and patient-centered care improvement.
