COLORADO COMMUNITY MANAGED CARE NETWORK

Colorado Community Managed Care Network (CCMCN) is a Colorado-based nonprofit membership network that supports community health centers and safety-net providers by providing data infrastructure, value-based care strategy, and clinical and operational coaching. CCMCN operates as the continuing legal entity and has unified operations and branding with Colorado Health Providers Alliance under the Carina Health Network identity to broaden services for member organizations. In recent years CCMCN has expanded its data and value-based care work: the network reported annual program revenues in the mid-single millions and has been an active participant in CMS value-based initiatives, achieving measurable Medicare Shared Savings performance in 2019–2024 and reporting record savings for PY2024. The organization also continues to secure federal grants and cooperative agreements to advance data sharing and care coordination across Colorado.
